Queen Elizabeth Hospital announces sentinel event

The following is issued on behalf of the Hospital Authority:

     ​The spokesman for Queen Elizabeth Hospital (QEH) announces a sentinel event today (September 16):

     A 60-year-old female patient was sent to QEH Accident and Emergency Department for treatment and admitted to a special observation ward yesterday (September 15) due to injury on left hand and mental confusion.

     As the patient has self-harm intention, healthcare staff applied safety vest and put on limb holder straps for the patient. The patient was under close monitoring.

     The patient was last seen by healthcare staff at around 3am this morning, who was in a stable and calm condition. A ward staff found the patient had a bandage coiled around her neck at around 3.30am. Healthcare staff removed the bandage and performed resuscitation immediately. The patient was certified dead at around 4.30am this morning.

     The hospital has reported the case to the Police immediately and will fully co-operate with the Police investigation. The hospital was saddened by the incident, expressed deep condolences to the family and will provide necessary assistance.

     The hospital is very concerned about the incident and has reported to the Hospital Authority Head Office via the Advance Incident Reporting System. The incident has been referred to the coroner for follow-up.
