Infirmary Units in Residential Care Homes for the Elderly
Introduction
Infirmary Units are an integral part of some subvented care and attention homes with a provision of additional nursing staff. They maintain and support elders who are assessed to be so chronically ill or disabled as to require infirmary service, so that the elders may remain in their existing homes. They may also help to tide over the elders for admission to medical infirmaries if the elders so wish. There are currently 19 subvented care and attention homes providing Infirmary Units with a total capacity of 559 places.
Nature of Service
Infirmary Units in care and attention homes provide constant nursing care, some medical attention/consultation as well as supporting therapeutic services for residents.
Eligibility Criteria
- Existing residents of subvented care and attention homes or subsidised places in private homes participating in the Enhanced Bought Place Scheme;
- Certified by Community Geriatric Assessment Teams (CGAT) of the Hospital Authority to be in a condition requiring infirmary placement. (Cases assessed by Community Geriatric Assessment Teams to be not in need of infirmary service may also be considered for admission to Infirmary Units if interRAI-HC assessment results indicate a service option of 'Beyond Nursing Home'); and
- currently not registered for Infirmary Care Supplement.
Application Procedure
In respect of residents of subvented care and attention homes, referrals for admission to Infirmary Units may be made by the social workers of the homes concerned. For residents occupying subsidised places in private homes participating in the Enhanced Bought Place Scheme, referrals may be made by casework service units such as Integrated Family Service Centres / Integrated Services Centres and Medical Social Services Units.

Higher Care Supplement
Infirmary Care Supplement (ICS)
ICS was introduced to help subvented residential care homes to take care of frail residents who have been medically assessed to be chronically ill or disabled requiring medical infirmary placement. ICS has been extended to private homes participating in the Enhanced Bought Place Scheme.
Multi-disciplinary Outreaching Support Teams for the Elderly (MOSTE)
Introduction
SWD launched Multi-disciplinary Outreaching Support Teams for the Elderly (MOSTE). Comprising social workers, physiotherapists, occupational therapists and speech therapists, MOSTE provides outreach support services for the service users of private residential care homes for the elderly (RCHEs) to address their social and rehabilitation needs. MOSTE also provides speech therapy service by speech therapists for the service users of contract RCHEs (including the Day Care Units (DCUs) attached to contract RCHEs) as well as self-financing RCHEs and self-financing Nursing Homes (NHs) who have swallowing difficulties or speech impairment.
