Community Rehabilitation Network

Description

To assist people with chronic illnesses to establish a supporting network in the community and to live a meaningful life through the provision of on-going support in the form of educational and training programmes, social networking and social support services. 

Eligibility for the Service

• People with chronic illnesses (such as cardiac disease, renal failure, diabetes and haemophilia); or
• Families, relatives and friends of people with chronic illnesses.

Application Procedures

Application can be made directly to the Community Rehabilitation Network. 

Fees and Charges

The membership fee is $21 per annum.
